The Bedford-Stuyvesant Family Health Center (BSFHC) is a Federally Qualified Health Center (FQHC) that serves all of the primary health care needs of families in the heart of North and Central Brooklyn for over 40 years. We are seeking a Quality Improvement & Risk Management (QI/RM) to join our team. QI/RM provides leadership and expertise to ensure the delivery of safe, high quality, patient care services at the Bedford Stuyvesant Family Health Center. This includes the development, planning, coordination and administration of a systematic program designed to reduce risk and injury.  QI/RM uses informatics tools and techniques of Continuous Quality Improvement, including analysis and interpretation of data using computer based disease registries (eClinicalWorks) and the Center for Primary Care Informatics data warehouse.

ESSENTIAL JOB RESPONSIBLITIES AND DUTIES:

Duties and Responsibilities include but not limited to:

The Quality Improvement and Risk Manager (QI/RM) functions as the lead manager, coordinator, and point of contact for quality improvement and risk management policies and activities.  As such, the manager provides systematic efforts aimed at enhancing the delivery of healthcare services and improving patient outcomes. It involves a structured approach to identify inefficiencies, reduce errors, and optimize patient care through data-driven methods. This continuous process seeks to standardize practices, achieve predictable results, and enhance the overall quality of care provided by healthcare organizations

  • Develop and continually refine methods for working with healthcare provider staff to implement effective and timely interventions that improve performance on quality indicators. Maintains an effective reporting system to communicate and document findings, action and follow-up resulting from systematic monitoring and evaluation criteria.
  • Works closely with leadership and payers on data analysts in the design of reports required to monitor provider and department performance and patients outcomes
  • Ensure internal reports and required CMS reports related to assigned providers are completed and submitted on time.Participates in quality management projects and initiatives.

  • Utilization of internal assessments and external benchmarking to proactively evaluate risk and provide corrective actions.
  • Encourage the event reporting process, trend and analyze results, identify problematic areas, and develop prevention initiatives.
  • Chair monthly QI/Risk management committee meetings and provide support to subcommittees as necessary.

EDUCATION/EXPERIENCE REQUIREMENTS:

  • Bachelor's Degree required.
  • 1-3 years of relevant experience, required.

Other Related Skills/Experience and requirements:

  • Familiar with PCMH recognition process and HEDIS/QARR measures. 
  • Experience working with disadvantaged populations helpful and knowledge of health disparities highly desired.

Salary: $25/hour and based on experience
