Description

The Bedford-Stuyvesant Family Health Center (BSFHC) is a patient-centered, team based multi-specialty practice located in central Brooklyn.  BSFHC has been providing affordable, comprehensive, outpatient medical and dental services in an outpatient setting for over 40 years.  As a Federally Qualified Health Center (FQHC), BSFHC serves all of the primary health care needs of families in the heart of North and Central Brooklyn. We have an opportunity for a Licensed Master Social Worker -  LCSW to provide quality, competent, safe and compassionate care based on established standards in a patient-focused preventative, treatment care and counseling. 

Bilingual Spanish preferred

A PLUS - Knowledge of the DSM –V, understanding of psychosis, OCD, and Substance Use Disorder Counseling
. Recovery oriented and trauma informed. Motivational Interviewing techniques, Cognitive Behavioral Therapy, and Behavioral Activation.

Duties and Responsibilities include but not limited to:

  • Perform intake assessments for patients new to the clinic.
  • Serving as a liaison between intra-departments that include nursing, medical records, patient relations and the patients/family and other service entities among the continuum of care;
  • Assessing and referring patients for appropriate behavioral care, and serves as a liaison between departments, including medical, nursing, medical records, patient relations, patients/family and other service entities among the continuum of care;
  • Maintaining detailed, updated, comprehensive records of daily activities, reports filed, received and/or pending, and always being current in documenting in the Electronic Health Record all interventions/services that were provided.
  • Providing crisis intervention to patients with an emergency (including referral to food pantries, emergency housing assistance, drug treatment, shelters and domestic violence services);
  • Providing short-term counseling to help patients adjust to new physical/mental diagnoses, focusing on helping patients to comply with establish treatment plans while offering a level of professional and moral support during the adjustment period and service coordination;
  • Documenting and reporting all cases of suspected child abuse/child neglect cases to Administration for Children Services.
  • Provide direct treatment services to patients within the clinic utilizing appropriate modalities including individual therapy, group therapy, and crisis intervention.
  • Act as advocate for the patient with other Medical Center programs (e.g., Outpatient department), and functions as liaison with other providers in the community.
  • Provide presentations on pertinent clinical topics to trainees and fellow clinicians.
  • Participate in interdisciplinary diagnostic and treatment planning conferences, including case presentations, and make recommendations regarding the needs of the patient from a social work perspective.
  • Provide clinical supervision to social worker (BSW/LMSW) as assigned.
  • Maintain established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards.

 

EDUCATION/EXPERIENCE REQUIREMENTS:

  • Masters in Social Work from an accredited school of social work.
  • Current NYS licensure as a LCSW.
  • 1 year of experience in outpatient psychiatry preferred.
  • Good interpersonal verbal/written communication skills.

Other Related Skills/Experience:

  • Experience/An understanding of eClinical Works is helpful.
  • The ability to demonstrate teamwork and effective interpersonal relations skills is imperative.
  • Excellent written/oral communication and computer skills are required.
